Benefits
EMPLOYEE BENEFITS SUMMARY
MDEA offers a "flexible benefits" program that provides employees the opportunity to select benefits with coverages that will meet their individual needs.
Benefits (with the exception of retirement savings "401k") become effective the first day of the month following the date of hire. For example, a person starting on January 12th would be eligible for benefits effective February 1st.
Employees are provided with flexible dollars to "purchase" the benefits and coverage levels best suited for their needs. Enough flexible dollars are provided to cover the full cost of many options. Choosing higher levels of some options may cost a small amount via payroll deduction. Choosing lower levels of coverage may leave employee with left-over flexible dollars to spend on other options. Left-over flexible dollars may also be directed to the 401(k) plan or taken as taxable cash back through payroll checks.
HEALTH INSURANCE
AETNA Indemnity Plan
- There are two levels of coverage. The differences among the options are in the deductible and the annual maximum out-of-pocket expenses. Both offer PPO options to reduce the expense to the employee. After the deductible is met, the plan reimburses 85% of the eligible expenses from preferred providers.
- Health Maintenance Organizations (HMO)
These require co-payments but no deductible. Depending on which state employee resides, MDEA offers several HMO plans including Kaiser (CA), PacifiCare (CA), Blue Cross Blue Shield (GA), Health Alliance (MI), United HealthCare (OH), Aetna (IL, NJ, TX), Harvard Pilgrim (MA)
- No coverage option. If you certify you have medical coverage from another source, you may elect no medical coverage and spend your company flexible dollars on another option or direct them to a spending account, 401(k) plan or taxable cash back.
DENTAL INSURANCE
- AETNA Dental. This indemnity plan covers 100% preventive services and 80% basic and major expenses after a $25 deductible. Annual maximum coverage of $2,000 per plan year for basic and major expenses per person. Orthodontia coverage is $2,000 lifetime maximum per person. Those insured under this plan may see the dentist of their choice. This is a two-year enrollment period.
- No coverage option. Proof of other dental coverage not required as with medical plan.
VISION INSURANCE
- Vision examination by a VSP physician is covered at 100% after $20 annual co-pay. One pair of glass lenses every 12 months and frames every 24 months. Contact lens benefits are provided in lieu of other vision benefits.
LIFE INSURANCE
MDEA provides $20,000 worth of life insurance coverage for each employee at no cost to employee.
ADDITIONAL LIFE INSURANCE
Supplemental life insurance is available to each employee. Employee pays the cost of coverage equal to one times their covered annual earnings. Optional life insurance amounts rounded to the nearest $1,000. Cost may be paid by flexible dollars or by employee depending on which options selected in other benefits choices. Evidence of Insurability is required in excess of one level increase at annual open enrollment.
ACCIDENTAL DEATH & DISMEMBERMENT INSURANCE
Coverage provided under life insurance policy.
LONG TERM DISABILITY INSURANCE (LTD)
Select coverage at 50% or 66-2/3% income continuance. Benefits begin after 180 days total disability.

At that time, employee may also elect to contribute “pre-tax” earnings into the plan up to a maximum of 25% of earnings, 4% of which MDEA will match (dollar for dollar).
